DISCLAIMER: SINCE HURRICANE HARVEY WE HAVE MADE CHANGES TO THE DIABETIC PROGRAM AND THESE ARE THE NEW CLIENT REQUIREMENTS!

Mission statement: We fight food insecurity and promote improved health through Diabetic nutrition education focused on teaching clients how to maximize community resources and proper nutrition.

New Client Requirements for the Southeast Texas Food Bank Diabetic Program:

-Clinically diagnosed Type I & II Diabetics, pre-Diabetics, or Diabetes prevention are all encouraged to apply

-Must attend all three FREE Diabetic Nutrition workshops to receive the box of food

-Provide clinical A1c record no later than second workshop

-Supplies to check blood sugar (glucose meter)

-Keep track of glucose 3 times a week for first 3 months of program

-Bring glucose tracking record when picking up monthly Healthy Food Box

-A1c’s due 4 times a year (every 3 months), or 2 times (every 6 months) if on maintenance or pre-diabetic
